If Paria Fuel Trading Company Limited had been constantly monitoring works being done by LMCS on February 25 at its Berth 6 riser, the tragic accident which cost four men their lives would not have happened.
This from Paria’s health, safety and environment technician, Andrew Dopson.
Dopson, who is sub-contracted by Paria through Kenson, agreed with counsel for the Commission of Enquiry, senior counsel Ramesh Lawrence Maharaj that if Paria had constant monitoring of the video screen in the hyperbaric chamber, this tragedy could have been prevented.
Four LMCS divers, Kazim Ali Jnr, Rishi Nagassar, Fyzal Kurban and Yusuf Henry lost their lives in the accident, which is largely believed to be have been caused by the removal of the inflatable plug on Paria’s 30-inch pipeline, which caused a differential pressure situation and sucked the men into the pipeline.
Dopson gave his testimony during the Commission of Enquiry set up to investigate the deadly accident on Wednesday morning.
