Re­mem­ber, the Ap­pli­ca­tion Form MUST al­so be com­plet­ed by a med­ical prac­ti­tion­er who is a Reg­is­tered Med­ical Spe­cial­ist in the Med­ical Spe­cial­ist Reg­is­ter un­der the Med­ical Board Act, Chap. 29:50

Ve­hi­cle own­ers and mo­torists, en­sure that you get your ve­hi­cle’s tint lev­els test­ed at a rep­utable tint shop, be­fore De­cem­ber 31st, 2021.
